Teeth Whitening

The reason teeth darken is the accumulation of various staining agents within them – pigments, organic molecules, tetracycline antibiotics, coffee, tobacco, soft drinks, tea, red wine, dyes, and other substances within the internal structure of the teeth. In cases of root canal treatment, teeth can become discolored to a reddish-brown, and sometimes almost black.

There is a significant difference between teeth whitening and cleaning them from plaque and tartar. Ultrasonic and air-flow cleaning always precedes any whitening procedure, as it removes accumulated plaque and tartar from the tooth surfaces, allowing the whitening agent to penetrate the deeper layers of enamel where pigments are deposited. There are mainly three types of whitening:

  • In-office whitening – This method uses 30–35% carbamide peroxide gel. After protecting the gums with a light-cured composite, the whitening gel is applied and activated with a light source. After whitening, the gel is rinsed off or removed with a suction device. The procedure can be repeated immediately, but no more than 3–4 times. The effect is noticeable immediately after the 1–1.5 hour procedure, and it lasts approximately 2–3 years.
  • At-home whitening – This method uses carbamide peroxide gel in a 15–20% concentration. Impressions of both jaws are taken to create precise silicone trays. A specified amount of gel is placed in the trays, which are then worn on the teeth for 4–6–8 hours, often during sleep. The procedure is carried out daily for 14–16 days. For subsequent treatments, the same silicone trays can be reused. Whitening effects accumulate daily and are typically visible after 5–6 days. Treatment can be extended for a few additional days for enhanced results. Patients should visit the dentist every other day during this process to prevent potential gum complications (as the gel is in prolonged contact with the gums) and to monitor the whitening effect using the VITA scale. The results usually last about 2–3 years.
  • Whitening of non-vital teeth – After removing the root canal filling material that caused the discoloration, a whitening gel is placed inside the tooth and activated with light until the desired shade is achieved. Once this is done, the tooth is restored with a filling.

When whitening is performed according to the manufacturer’s instructions, no negative effects are typically observed. Occasionally, increased tooth sensitivity may occur, lasting from one to several days. Treating the whitened teeth with a fluoride-containing gel helps prevent or minimize such complications.

Bonding

Tooth bonding is a popular dental procedure used to improve the aesthetic appearance of teeth. By shaping new forms, surfaces, and contours directly on the tooth with composite material, impressive results can be achieved in just one visit. Bonding is a quick solution for various issues, such as:

  • Closing gaps between teeth: Bonding can fill spaces between teeth to create a more attractive smile.
  • Shaping teeth: If teeth have an uneven or undesired shape or size, bonding can help achieve a more aesthetically pleasing form.
  • Covering stains: Bonding can conceal unattractive stains or discolorations, giving teeth a fresher and cleaner appearance.
  • Repairing chips: Small chips or damages to teeth can be restored with bonding to maintain integrity and prevent further damage.
  • Tooth lengthening or widening: The procedure allows increasing the length or width of teeth.

Bonding is a painless procedure and usually does not require anesthesia, except in cases of more extensive restorations. Maintaining bonded teeth requires regular dental visits and good oral hygiene to preserve their appearance and function. In conclusion, tooth bonding is a safe and effective solution for a variety of dental issues, offering immediate results and boosting self-confidence.
